Q. How many
employees do I need to qualify for coverage?
A. At least 2 and no more than 50 full-time (at least 30 hours per week)
worldwide employees for at least 50% of the previous calendar quarter from the effective
date.
Q. Do my employees need to reside in a
specific area?
A. All new membership is limited to those individuals who live within our
Service Area. Check our Service Area ZIP Codes to make sure your employees qualify.
Q. Do I have to pay the entire premium
for my employees?
A. Your contribution must be at least 50% of the Kaiser Permanente rate
for single subscribers. Any part of the cost not paid by the employer must be collected
from employees through payroll deduction.
Q. How do I enroll?
A. To start the enrollment process, just complete the Enrollment
Application Request Form. An enrollment packet will be sent to you within 7-10

Q. I have a Kaiser Health
Insurance plan as an individual. Can I convert to a Group Health
Insurance Plan with your program?
A. Yes, please complete the on-line
